When a user uploads an image, they click Select Photo, pick the image and before clicking on save, it shows a broken icon;

I just gave it a try, entered 'image icon' and this is the result:
The same globally known symbol used equally by Ossn. Mountains and a sun supposed to represent a landscape and not something broken.

Looks like broken and actually broken are two different things.
I don't see anything broken in the image you shared.

Right below the 'Select Photo'.
It looks like a broken icon even if it isn't.
